Build Greater Capacity for Culturally Safe Care: Primary care providers and front-line health healthcare workers will enhance their capacity to provide culturally safe & affirming care to Black and racialized patients
Primary care providers and frontline healthcare workers will learn to recognize and understand the unique clinical presentations and challenges faced by Black & racialized individuals in relation to prostate cancer, sickle cell disease, dementia, cardiovascular disease, and GFR and kidney function.
Strengthen Collaborative Care and Counseling: Family physicians and other healthcare providers will learn strategies to effectively support Black and racialized patients managing complex health conditions alongside specialist intervention. This includes enhancing their ability to counsel patients on treatment options, lifestyle changes, and ongoing management of their health in collaboration with specialists.
